Autor/es o nombre de la entidad responsable. Título seguido del Tipo de medio entre paréntesis cuadrados "[ ]". Edición. Lugar de publicación: Editorial, año. Extensión del documento y medidas.

Ejemplo:

 

PRACTICAL body composition kit [Multimedia]. Champaign (Illinois): Human Kinetics, Corp. 1995. 1 guía (74 p.), 1 videocasete (VHS, color, 17 min.), 1 disquete 3" 1/2, 1 calibrador, 1 cinta métrica. Todo el material en un estuche.
